Dr. Gamarra reviews your history and symptoms, performs an examination, and leaves time for your questions. Hablamos Espanol.
Care Plan
Together you choose a plan that fits your needs, from in-office procedures to robotic surgery, with follow up care to support your recovery.
What to Bring
Photo ID and Insurance Card
A government-issued photo ID and your current insurance card, if you have one.
Medication List
Names and doses of prescriptions, over-the-counter medicines, and supplements you take, plus any allergies.
Prior Records
Imaging, lab results, operative reports, or notes from other providers related to your concern.
Emergency Contact
Name and phone number of someone we can reach if needed.
Patient Forms
Forms available at our office or can be completed at your first appointment.
Intake form. Your medical history, current symptoms, and medications.
Consent form. Your agreement to evaluation and care after your questions are answered.
HIPAA authorization. Your privacy preferences and who may receive your health information.
